Text For Citation: 01 Item/Group: 001 Hazard:
Section 5(a)1 of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to crushing hazards associated with tilt-frame waste collection equipment: a) Cotterman & Sons LLC on or about December 02, 2013, at the address of 3700 Kolbe Rd. Lorain, OH: An employee removing a 40 cubic yard waste container from a construction site, via a tilt-frame waste collection vehicle was exposed to crushing hazards after climbing underneath the partial hoisted container in an attempt to dislodge entangled roofing material from the container's latching mechanism. Among other methods, one feasible and acceptable method to correct this hazard is to follow the American National Standard Institute (ANSI) Z245.1 - 2008, Mobile Wastes and Recyclable Materials Collection, Transportation, and Compaction Equipment-Safety Requirements: 1) Section 6.1.3 -Assign only trained employees to operate waste collection equipment (specifically 6.1.7.4.1 -container lifting training). 2) Section 6.1.4 -Monitor employees' activities and taking appropriate action to ensure adherence to safe work practices and the employee requirements of this part. 3) Section 9.2.1 -Develop a safety program that includes a hazard assessment and methods of control in written form.
