Text For Citation: 01 Item/Group: 001 Hazard:
OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck-by traffic hazards. Site: Columbus Avenue & West 67th Street New York, NY On or about 11/5/13 a) Trailer struck an aerial boom lift, the mployees working on the boom lift were thrown off the boom's basket. AMONG OTHER METHODS, FEASIBLE AND ACCEPTABLE ABATEMENT METHODS TO CORRECT THIS HAZARDS ARE: 1. Train employees on the appropriate flagger procedures as per the Manual on Uniform Traffic Control Devices (MUTCD) section 6E.01 (page 566). Flaggers are responsible for public safety and make the greatest number of contacts with the public of all highway workers, they should be trained in safe traffic control practices and public contact techniques. 2. Employees should set up the lane merging taper according to the proper taper length calculation as per the Manual on Uniform Traffic Control Devices (MUTCD) section 6C.05 (page 554) Transition Area, which states the when the redirection of road users normal path is required, they shall be directed from the normal path to the new path. Guidance is listed in Table 6C-3 (page 557) "Taper Length Criteria for Temporary Traffic Control Zones" and 6C-4 (page 557) "Formulas for determining Taper Length". They were to use 6 cones every 30 feet.
