29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: (a) At the MARTA maintenance yard located at 2126 West Point Avenue in College Park, Georgia, the employer had developed and documented a LOTO program but failed to utilize the procedures to isolate all energy sources for the LRG14 - Light Rail Grinder to prevent the unexpected energizing or release of store energy of the grinding modules, exposing employees to struck-by and electrical hazards.
