29 CFR 1910.146(j)(4): The employer did not ensure that each entry supervisor verified that rescue services were available and that the means for summoning them were operable: a. On or about September 20, 2013, an employee entered liquid AC tank number three, a permit required confined space, to cut out a section of pipe, exposing the employee to atmospheric, heat, thermal burns, and entrapment hazards. The plant foreman, entry supervisor, did not verify that the Jacksonville Fire and Rescue Department was available to respond to an emergency situation in the confined space to perform rescue services before the employee entered the tank.
