29 CFR 1910.146(e)(2): Before entry begins, the entry supervisor identified on the permit shall sign the entry permit to authorize entry: a. On or about September 20, 2013, an employee entered liquid AC tank number 3, a permit required confined space, to cut out a section of pipe, exposing himself to atmospheric, heat, thermal burns and entrapment hazards. The entry supervisor, plant foreman, did not sign the entry permit before entry was made by the entrant.
