29 CFR 1910.146(e)(1): Before entry was authorized, the employer did not document the completion of measures required by 29 CFR 1910.146(d)(3) by preparing an entry permit: a On or about September 20, 2013, an employee entered liquid AC tank number 3, a permit required confined space, to conduct servicing and maintenance operations, exposing himself to potential hazards such as but not limited to atmospheric, heat, severe thermal burns and entrapment. The plant foreman/entry supervisor, failed to document the completion of measures required by 29 CFR 1910.146(d)(3), and failed to complete the entry permit prior to allowing the employee to enter liquid AC tank number 3, in that the entry supervisor had not: 1) Identified the space to be entered, i.e. liquid AC tank number 3, in accordance with 29 CFR 1910.146(f)(1); 2) Identified the purpose of the entry into the confined space, i.e. liquid AC tank number 3, in accordance with 29 CFR 1910.146(f)(2); 3) Identified the authorized duration of the entry permit in accordance with 29 CFR 1910.146(f)(3); 4) Identifed and evaluated the hazards of the permit space and documented on the permit in accordance with 29 CFR 1910.146 (f)(7); 5) Isolated the permit space in accordance with 29 CFR 1910.146(d)(3)(iii) and document on the permit in accordance with 29 CFR 1910.146(f)(8); 6) Verified that conditions in the permit space were acceptable for entry in accordance with 29 CFR 1910.146(d)(3)(vi) and document on the permit in accordance with 29 CFR 1910.146(f)(9); 7) Identified and documented the results of all of initial atmospheric tests performed in accordance with 29 CFR 1910.146(f)(10); 8) Identified the personal protective equipment and testing equipment to be used, in accordance with 29 CFR 1910.146(f)(13);
