Text For Citation: 01 Item/Group: 002A Hazard:
29 CFR 1926.451(a)(1): Scaffold(s) and scaffold(s) component(s) were not capable of supporting, without failure, its own weight and at least four times the maximum intended load applied or transmitted to it: for the ladder jack scaffold system located at 950 Cherry Hills Drive, Sun City Center (FL) used by employees to strip paint from a sheet metal gazebo roof, on or about 08/07/2013. As observed on or about 08/15/2013 the ladder jack scaffold system and components had various deficiencies in the design and setup that affected the capacity of the system: a) the width of the platform was 11.25 inches instead of 12 inches as required by 29 CFR 1926.451(b)(2)(i) b) the working width of the platform was further reduced by installing the brackets on the inside of the ladder such that the platform was nearly under the edge of the roof. c) the span of the ladder jacks supports was 12 feet instead of 8 feet as per 1926 Subpart L App A - Scaffold Specifications. d) the work platform was sandwiched between the ladder jack support clamps instead of fastened and secured.
