29 CFR 1910.178(l)(4)(ii)(E): The employer failed to evaluate when a condition in the workplace changed in a manner that affected safe operation of the powered industrial truck: (a) Vail Resorts, Tower 36, Chair A, 1599C Summit County Road 3, Breckenridge, CO 80424: The employer failed to perform refresher training on the Lull All-Terrain Forklift when the normal procedure for rigging or lifting the haul rope off the cable was not available. This condition resulted iin an employee falling from a height of 21'.
