Text For Citation: 01 Item/Group: 002A Hazard:

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck-by hazards: Vail Resorts, Tower 36, Chair A, 1599C Summit County Road 3, Breckenridge, CO 80424: The employer used a sling wrapped around the tyne of a Lull All-Terrain Forklift to lift the haul rope off its sheave assembly. On the morning of 7/11/11, one employee fell from Tower 36, Chair A while on a Riblet Lift when the sling holding the haul rope above the eveners severed against the sharp edges of the forklift tyne and dropped the cable and work chair to the ground. This condition exposed employees to struck-by hazards. Abatement Note: Among other methods, one feasible and acceptable abatement method to correct this hazard would be to research and contact an additional source who has performed the same task on the same kind of lift in a safe manner. Abatement Note: ASME B30.9-2010, 9-3.10.4 (d) Rigging Practices- Slings in contact with edges, corners, or protrusions should be protected with a material of sufficient strength, thickness, and construction to prevent damage. Abatement Note: Abatement certification and documentation are required for this item (see enclosed Certification of Corrective Action Worksheet).
