Text For Citation: 01 Item/Group: 001 Hazard:

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to fall hazards: Vail Resorts, Tower 36, Chair A, 1599C Summit County Road 3, Breckenridge, CO 80424: The employer did not perform a job hazard analysis of a new procedure or document their own procedures when lifting the cable above the eveners to inspect and perform maintenance on the sheaves. On the morning of 7/11/11, an employee fell from tower 36, Chair A while perofmring maintenance on a Riblet lift. The haul rope which was supporting the employee's work chair was severed by the tyne of a forklift. This resulted in an employee falling approximately 21 feet to the ground. Abatement Note: Among other methods, one feasible and acceptable abatement method to correct this hazard is to perform a job safety (hazard) assessment prior to performing any new rigging operation. Note: Abatement certification and documentation are required for this item (see enclosed Certification of Corrective Action Worksheet).
