29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: The employer failed to meet the requirements of this paragraph of the subpart in that they are failing to ensure that employees are using a ladder in accordance with the standards outlined in the regulation. The most recent example of this was found on the job site located at 15110 Sherwood Ave, Omaha, NE 68116 where the employer has employees roofing a new residential home with ground eave heights over 9 and roof pitch greater than 4-12; using an extension ladder as a means of access and the ladder does not extend the minimum 3 above the walking/working surface.
