29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: (a) A Monarch Machine Tool Co. manually operated lathe was not provided with a chuck guard to protect employees from rotating parts creating an entanglement hazard. (c) On or about 6/25/13, an employee was polishing a metal shaft with a piece of loose emery cloth, creating an entanglement hazard caused by the rotating part in conjunction with the loose cloth. The employee's left hand was pulled into the rotating shaft, resulting in the amputation of the employee's arm below the elbow.
