29 CFR 1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress were not provided in trench excavations that are 4 feet (1.22 m) or more in depth so as to require no more than 25 feet (7.62 m) of lateral travel for employees: a) South wall of house, 1429 Deepwood Road, Pittsburgh, Pennsylvania 15241: Observed on or about June 28, 2013, employees were working in an unprotected trench 9 feet in depth with unprotected sidewalls, without a stairway, ladder, ramp, or other safe means of egress. The employees were exposed to crushed-by and engulfment hazards. The MWA Acquisitions was previously cited for a violation of this occupational safety and health standard or its equivalent standard (29 CFR 1926.651(c)(2)), which was contained in OSHA inspection number 315561662, citation number 01, item number 001 and was affirmed as a final order on November 29, 2011, with respect to a workplace located at 630 Ray Drive, Carnegie, Pennsylvania 15106.
