29 CFR 1910.178(a)(4): Modifications and additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval: a) On or about July 22, 2013, the employer failed to assure that approval was obtained by the manufacturer before adding a working platform to a Caterpillar forklift (model C6000 LP; s/n AT83F20144). The addition was used in the ACO building to lift maintenance workers.
