29 CFR 1910.219(e)(2)(iv): Overhead chain and link belt drives, governed by the same rules as overhead horizontal belts, were not enclosed by guard(s) in the same manner as belts conforming to the requirements specified at 29 CFR 1910.219(m) and (o): (a) On or about May 30, 2013, and at times prior, for the employees who operated the blenders where approximately six inches of access to the pulleys and chain, due to insufficient guarding on the back, exposed employees to caught-in and amputation hazards in Building 2, the Gluten-free Packaging Room at 500 North Street West in Harvey, North Dakota. (b) On or about May 30, 2013, and at times prior, for the employees who accessed the platform on top of the milling machine, approximately nine feet above the ground, in order to perform maintenance and repair or to change out screens on the milling machine in Building 9, the Gluten-free Milling Room at 500 North Street West in Harvey, North Dakota.
