Text For Citation: 01 Item/Group: 001 Hazard:
29 CFR 1910.36(d)(1): Employees were not able to open an exit route door from the inside at all times without keys, tools, or special knowledge. A device such as a panic bar that locks only from the outside is permitted on exit discharge doors. (a) Ammonium Polyphosphate Portable Pipe Reactor, on or about May 18, 2013, and at times prior thereto, the employer did not ensure that the exit was unlocked when operators were inside. Employees were unable to open the single exit door, secured by a deadbolt lock, in the case of an emergency such as an ammonia release. Note: Abatement certification and documentation are required for this item.
