29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with paragraph (b) or (c) of this section: A. VTF Excavation, LLC worksite located near 207 S. Broadway, Columbus Grove, Ohio: On or about May 22, 2013, the employer did not ensure an employee was adequately protected from cave-ins while installing a sanitary sewer structure in the north end of the trench. An employee was working within stacked manhole boxes in a trench which was approximately 16.00 feet deep. No cave-in protection was provided on the north end of the manhole box. VTF Excavation, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.652 (a)(1), which was contained in OSHA inspection number 313781072, citation number 1, item number 2 and was affirmed as a final order on February 18, 2010 with respect to a workplace located at Celina, Ohio.
