Text For Citation: 01 Item/Group: 006 Hazard:
29 CFR 1910.146(k)(1)(i): An employer who designated rescue and emergency services, pursuant to paragraph (d)(9) of this section, did not evaluate a prospective rescuer's ability to respond to a rescue summons in a timely manner, considering the hazard(s) identified: a. Howard F. Curren Advanced Wastewater Treatment Plant - for employees conducting a horizontal confined space entry into an 8 ft diameter sewer pipe through the 90 degree bend opening leading to meter building and the opening leading to sedimentation tanks #5-8, on or about 4/11/13. The entry permits identified emergency rescue numbers as "911"; however, the employer did not coordinate and/or evaluate the prospective rescuer's ability to perform rescue. Employees were exposed to hydrogen sulfide gas. b. Howard F. Curren Advanced Wastewater Treatment Plant - for employees conducting a horizontal confined space entry into an 8 ft diameter sewer pipe using the ladder near the sedimentation tanks to enter the confined space to install a vacuum hose on or about 4/10/13. The entry permits identified emergency rescue numbers as "911"; however, the employer did not coordinate and/or evaluate the prospective rescuer's ability to perform rescue. Employees were exposed to hydrogen sulfide gas. c. Howard F. Curren Advanced Wastewater Treatment Plant - for employees conducting a vertical confined space entry into the junction chamber to work with the stop logs, on or about 4/9/13. The entry permits identified emergency rescue numbers as "911"; however, the employer did not coordinate and/or evaluate the prospective rescuer's ability to perform rescue. Employees were exposed to hydrogen sulfide gas.
