29 CFR 1910.146(f)(10): The entry permit that documented compliance and authorized entry to a permit space did not identify the results of initial and periodic tests performed under 29 CFR 1910.146(d)(5), accompanied by the names or initials of the tester: a. Howard F. Curren Advanced Wastewater Treatment Plant - for employees conducting a horizontal confined space entry into an 8 ft diameter sewer pipe through the 90 degree bend opening leading to meter vault #2 -building #7, on or about 4/11/13. Employees entered the confined space without the atmospheric test results documented on the entry permit. This section of pipe was not provided with forced air ventilation. b. Howard F. Curren Advanced Wastewater Treatment Plant - for employees conducting a horizontal confined space entry into an 8 ft diameter sewer pipe through the 90 degree bend opening leading to sedimentation tanks #5-8, on or about 4/11/13. Employees worked in the permit required confined space for approximately 4 hours. Periodic atmospheric conditions were not recorded. Exit atmospheric conditions were not recorded on the entry permit. c. Howard F. Curren Advanced Wastewater Treatment Plant - for employees conducting a vertical confined space entry at Junction Chamber #2, on or about 4/9/13. Employees re-entered the confined space at 10:30 am without documentation of atmospheric test results on the entry permit. The employee exited at 11:05 am without exit readings documented on the entry permit. The entry permit lists the last exit reading at 9:02 am which is before the re-entry occurred. d. Howard F. Curren Advanced Wastewater Treatment Plant - for atmospheric readings at the opening to the pipe section leading to sedimentation tanks # 5-8, on or about 4/9/13. A 4-gas meter was lowered into the ladder opening and was used to measure the atmospheric conditions at approximately 12:05 pm at opening. The hydrogen sulfide levels were high and caused the gas meter alarm to activate. When the alarm activated the entry permit was voided; however, the atmospheric levels were not recorded on the entry permit to inform employees of the dangerous levels.
