29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights) more than 6-feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes: The employer does not ensure that each employee, who is working on a roof, is protected from falling through skylights which are more that 6-feet above lower levels . This violation most recently occurred on or about April 2, 2013, at a workplace located at 729 Third Avenue, Dallas, TX 75226; where at least one (1) employee, who was repairing/replacing skylights, was not protected from falling through the skylights and was exposed to a fall hazard of approximately 25 feet to the lower level.
