29 CFR 1926.452(a)(9): Where wooden poles were spliced, the ends were not squared and the upper section did not rest squarely on the lower section. Wood splice plates were not provided on at least two adjacent sides, and did not extend at least 2 feet (0.6 m) on either side of the splice, overlap the abutted ends equally, and have at least the same cross-sectional areas as the pole. Splice plates of other materials of equivalent strength may be used: (a) On the second level on or about February 11, 2013, the wood poles were overlapped and nailed together instead of being abutted, end to end with splice plates on a double-pole pole wood scaffold. The scaffold was about 50 feet long and the top platform was about 15 feet, 2 inches high. Employees were on the scaffold to build form work.
