29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energization, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative: (a) The employer did not have an established lockout/tagout program for employees who performed servicing and maintenance on machines and equipment such as but not limited to conveyors, pumps, mixers and block machine. Abatement Note: A lockout/tagout program should include the following key elements: - Energy source evaluation - Development, documentation and utilization of machine-specific procedures - Procedures for group lockout (when more than one employee is servicing the same equipment at the same time) - Training for "authorized," "affected," and "other"employees - Periodic inspections (at least annual) - Sharing of lockout/tagout program information with outside contractors There is no abatement documentation or certification required for this item.
