29 CFR 1910.303(c)(3)(i): Conductors were not spliced or joined with splicing devices identified for the use or by brazing, welding, or soldering with a fusible metal or alloy, soldered splices were not spliced or joined to be mechanically and electrically secure without solder and then soldered and all splices and joints and the free ends of conductors were not covered with an insulation equivalent to that of the conductors or with an insulating device identified for the purpose: Instances including but not limited to the following: a) On or about January 15, 2013, during shop vacuuming activities, the employer did not ensure that the electrical splicing for the cord that ran from above overhead lights to the ground below near the main office section of the building was apppropriately done.
