Summary of Work-Related Injuries and Illness, OSHA's Form 300-A (or its equivalent) for calendar year 2012. c. Injury and Illness Incident Report, OSHA's Form 301 (or its equivalent) for each recordable injury entered on OSHA's Form 300 (or its equivalent) for calendar year 2013.
