29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by an authorized employee: a) JBS USA, LLC at 800 North 8th Avenue, Greeley, CO 80631: On or about December 4, 2012, the employer did not ensure that an authorized employee applied energy isolating devices to two Cryovac machines when working on the Shrink Tunnel located between the two machines Condition exposed this employee to pinch point and amputation hazards.
