29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of energy control procedures at least annually to ensure that the procedures and the requirement of this standard were being followed: a) On or about 12/14/2012, In the foundry: Periodic inspections of lock out procedures did not evaluate and address potential deficiencies in the procedures and improper implementation of the procedures; such as, but not limited to identifying all potential energy sources, the use and limitations of using a single "prop rod" during maintenance of the "Spider". Energy sources not accounted for in the procedure for servicing, maintaining, or repairing the "Spider" include gravity. There was the potential for the unsupported raised "Spider" to drop while employees were working under the "Spider".
