29 CFR 1910.146(k)(1)(v): When designating rescue and emergency services pursuant to paragraph (d)(9), the employer did not provide the selected rescue team or service with access to all permit spaces from which rescue could be necessary, so that the rescue service could develop appropriate rescue plans and practice rescue operations: a) On or about November 29, 2012, Phoenix Industrial Cleaning, Inc., selected "911" as their designated rescue and emergency service during confined space entry. The employer did not provide this service with access to the permit space from which rescue could be necessary, so that the rescue service could develop an appropriate rescue plan and practice rescue operations prior to employee(s) entry into a 6,000 gallon 16 foot vertical chemical storage tank at Sunnyside Corporation in Wheeling, Illinois. This tank was last used by Sunnyside Corporation to store methylene chloride.
