29 CFR 1910.146(f)(10): The entry permit that documents compliance and authorizes entry to a permit space did not identify the results of initial and periodic tests performed under 29 CFR 1910.146(d)(5), accompanied by the names or initials of the testers and by indication of when the tests were performed: a) On or about November 29, 2012, Phoenix Industrial Cleaning, Inc. did not ensure the results of initial and periodic tests required under 29 CFR 1910.146(d)(5) were documented on the confined space entry permit, prior to and during employee(s) entry into a 6,000 gallon 16 foot vertical chemical storage tank at Sunnyside Corporation in Wheeling, Illinois. This tank was last used by Sunnyside Corporation to store methylene chloride.
