29 CFR 1910.146(d)(5)(i): Under the permit-required confined space program required by 29 CFR 1910.146(c)(4), the employer did not evaluate permit space conditions when entry operations were conducted by testing conditions in the permit space to determine if acceptable entry conditions exist before entry is authorized to begin: a) On or about November 29, 2012, Phoenix Industrial Cleaning, Inc. did not ensure permit space conditions were evaluated by testing conditions in the permit space to determine if acceptable entry conditions existed before entry by employee(s) was authorized to begin in a 6,000 gallon 16 foot vertical chemical storage tank at Sunnyside Corporation in Wheeling, Illinois. This tank was last used by Sunnyside Corporation to store methylene chloride.
