1910.146(d)(3)(vi): Under the permit-required confined space program required by 29 CFR 1910.146(c)(4), the employer did not develop and implement the means, procedures, and practices necessary for safe permit entry operations, including, but not limited to, verifying that conditions in the permit space are acceptable for entry throughout the duration of an authorized entry: a) On or about November 29, 2012, Phoenix Industrial Cleaning, Inc. did not develop and implement the means, procedures and practices necessary for safe permit space entry operations for employee(s), including, but not limited to, verifying that conditions in the permit space are acceptable for entry throughout the duration of an authorized entry, during entry into a 6,000 gallon 16 foot vertical chemical storage tank at Sunnyside Corporation in Wheeling, Illinois. This tank was last used by Sunnyside Corporation to store methylene chloride.
