29 CFR 1910.134(k)(1): The employer did not provide respirator training that would ensure each employee could demonstrate knowledge of items in section (i)-(vii): a) On or about December 26 and 27, 2012, Phoenix Industrial Cleaning, Inc. did not provide employees, who wore Racal Powerflow powered air purifying respirators (PAPRs) equipped with P100 filters, respirator training before the respirator was used by employees during cleaning of air handling systems serving the Pilot Plant and Production Area at URL Pharmpro, LLC in Aurora, Illinois. b) On or about December 17, 2012, Phoenix Industrial Cleaning, Inc. did not provide employees, who wore Racal Powerflow powered air purifying respirators (PAPRs) equipped with P100 filters, respirator training before the respirator was used by employees during cleaning of a pan coater exhaust system at URL Pharmpro, LLC in Aurora, Illinois. c) On or about November 29, 2012, Phoenix Industrial Cleaning, Inc. did not provide an employee, who wore a Sperian half-mask negative pressure air purifying respirator equipped with organic vapor/acid gas cartridges, respirator training before the respirator was used by the employee during permit space operations in a 6,000 gallon 16 foot vertical chemical storage tank at Sunnyside Corporation in Wheeling, Illinois. The tank entered was last used by Sunnyside Corporation to store methylene chloride.
