Text For Citation: 01 Item/Group: 001 Hazard:
Section 5(a)(1) of the Occupational Safety and Health Act of 1970 : the employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck by and/or crushing hazards from falling or out of control loads: Work Barge located to the south of Jennie K Tug Boat- During November 15, 2012, a 3 ton propeller was removed by side pulling with a 5 ton Coffing Hoist Model LHH which was being operated with a combination of side pulling, angled hoisting, and/or non-straight line loading of the load chains hook to hook. Feasible means of abatement include, but are not limited to: -Following the requirements of ASME B30.16 -2012 under sections 16-3.1 Operator Training and 16-3.3 Operation: Hoists shall be used to lift loads vertically without side pull except when specifically authorized by the manufacturer or a qualified person for the equipment and a qualified person for the supporting structure who have determined that: The various parts of the equipment, support system, and supporting structure will not be overstressed The stability of the equipment is not thereby endangered -Follow the requirements of the manufacturers LHH Series Operating Manual, specifically: Safety Precautions: 22. DO NOT operate a hoist when it is restricted from forming a straight line from hook to hook in the direction of loading. Safety Rules: c. Do not side load the hoist. Always pull in a straight line between hooks. Side loading over a sharp corner may facture the hoist housing, load block or hook. q. Hoists are designed for lifting loads vertically and should not be used for horizontal or angle hoisting.
