29 CFR 1915.89(d)(1): Procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, 29 CFR 1910.147(c)(4)(ii)(A) - (D): A.) DonJon Shipbuilding and Repair LLC, 220 East Bayfront Parkway, Erie, Pennsylvania 16507, on or about March 21, 2013 and times prior thereto: Employees were utilizing lockout procedures for the service of the 100 ton overhead crane and the press brake. The service on these pieces of machinery included but was not limited to, routine preventative maintenance. The lockout procedures utilized were missing information including, but not limited to, the intended use of the procedure; sources and magnitudes of energy; notification of affected employees prior to and following the lockout of equipment; specific procedural steps for shutting down, isolating, blocking and securing equipment; specific procedural steps for the placement, removal and transfer of lockout devices or tagout devices and the responsibility for them; specific requirements for testing equipment to determine and verify the effectiveness of lockout devices, tagout devices, and other energy control measures; and steps to release the lockout.
