Text For Citation: 01 Item/Group: 001 Hazard:
OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to rotating sucker rod while working aloft in the rod basket: On or about October 1, 2012, employees at the McBrayer 1-26 for Herman Loeb Well were performing oil well pump replacement operations. The derrick hand working aloft on the workover rig was exposed to caught-in hazards from the rotating sucker rod. Among other methods, some feasible and acceptable abatement methods to correct this hazard may include but are not limited to the following: 1. Develop, implement and enforce procedures requiring positive assurance from the derrick hand that he is clear of the rod prior to the floor hand utilizing the Power Sucker Rod Tongs to spin the sucker rod. 2. Ensure that signals, signs and/or other types of communication between supervisors, personnel and other involved persons are agreed upon and fully understood and utilized prior to initiation of operations as indicated in Paragraph 12.3 of the American Petroleum Institute Recommended Practice 54.
