Text For Citation: 01 Item/Group: 001 Hazard: STRUCK BY
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: a) The employees while performing their normal work activity were exposed to a freight elevator with a mission vision panel on the door exposing them to the hazards of serious injury or death by access to and being struck by the moving elevator exposed by the approximate 12 inch & 18 inch opening in the door, located on the sixth floor landing, Building #3, Elevator #8. Among other methods of feasible and acceptable abatement to correct the hazard as referenced in A.S.M.E. Standards Requirements A17.1-1960, Safety Code for Elevators, Section 110.11 is to replace and/or have replaced this missing vision panel (opening) and/or direct employees to use other available elevators (#9 & #10) that are properly protected. Violation observed on 2/3/89.
