Text For Citation: 01 Item/Group: 001 Hazard: CRUSHING
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to a crushing hazard in that: a) A positive means to lock out electrical power for the Piqua Engineering, Inc., Series 30/60 Industrial High Density Hydraulic Baler located in the back storage area of store Number 117, was not provided, employees were exposed to inadvertent operation of the ram (platen) during the tying sequence of the Baler operation, January 6, 1988. AMONG OTHER METHODS, A FEASIBLE AND ACCEPTABLE ABATEMENT METHOD TO CORRECT THIS HAZARD IS: 1) To provide Zero Mechanical State (ZMS), as to provide maximal protection against unexpected mechanical movement of equipment by locking out the electrical and mechancial power to the equipment. In addition, a written program for ZMS should be established with training of all affected personnel.
