29 CFR 1910.23(a)(2): Ladderway floor opening(s) or platform(s) were not provided with swinging gates or were not so offset that a person could not walk directly into the opening. On August 15, 2012, and at times prior thereto, in the oil pit under the automotive maintenance shop located on the south east side of store where oil changes were performed; employer did not provide platforms that were guarded to prevent falls. Employees were exposed to a 6 foot fall hazard.
