29 CFR 1910.119(c)(2): The employer did not consult with employees and their representatives on the conduct and development of process hazards analyses and on the development of the other elements of process safety management in this standard: On or about August 2, 2012, and at times prior thereto, the employer did not consult with Distillate Hydrotreater Unit operators regarding the cleaning and return to service of the 1E-101 charge/effluent exchangers while the unit was in operation exposing employees to the hazard of fire and explosion.
