29 CFR 1910.305(a)(2)(x): Flexible cords and cables were not protected from accidental damage: (a) At 635 Old Hwy. 90 West, Vidor, TX, Crown Section of the fabrication shop, employees were exposed to electric shock when using a E54864 12 AWG extension cord to energize a 120 volt grinder and fan. The flexible cord was exposed to hot electrical equipment (welding ground), hot slag, hot metal surfaces (from welding and cutting), pinching from heavy objects, etc.
