29 CFR 1910.304(b)(3)(ii)(A): All 125-volt, single-phase, 15, 20 and 30-ampere receptacle outlets that are not part of the permanent wiring of the building or structure and that are in use by personnel did not have ground-fault circuit-interrupter protection for personnel: (a) At 635 Old Hwy. 90 West, Vidor, TX, Crown Section of the fabrication shop, employees were exposed to electric shock when using a E54864 12 AWG extension cord to energize a 120 volt grinder and fan plugged into a Miller welding machine without a ground fault circuit interrupter (GFCI) for employee protection.
