Text For Citation: 01 Item/Group: 001 Hazard:
29 CFR 1910.269(l)(2): "Minimum approach distances." The employer shall ensure that no employee approaches or takes any conductive object closer to exposed energized parts than set forth in Table R-6 through Table R-10. At an Entergy jobsite located at 22 Carrington Circle (Southland Woods Division) the employer did not ensure employee(s) were prevented from violating minimum approach distances by allowing a conductive object to contact energized parts. This condition occurred on or about July 12, 2012, when an electric utility lineman used a 9-inch L-shaped Allen Wrench that had insulation removed on the end to make a final hot leg connection permanent service to a residence. An unprotected part of the employees body contacted the non-insulated section of the wrench which was energized with a current of 120 volts (240 volts phase-to-phase). The employee suffered an electrical shock from the contact while performing the wire connection task at the site that resulted in his death.
