Text For Citation: 01 Item/Group: 001 Hazard:
OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish to each of his employees and a place of employement which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to a crushing hazard. 1180 Mulberry Road, Houma, LA: On or about June 4, 2012, the employer did not ensure that safe work procedures were developed and implemented for employee(s) working on or around heavy objects. A 6,600 lb. steel wall fell on an employee as he was attempting to reposition a bearing on the wall. The employee was fatally injured. Among other methods, feasible and acceptable methods to correct this hazard are to: 1) Ensure that the wall is properly secured using bracing that is able to support the weight of the wall. 2) Implement written instruction outlining the perferred method of performing the task, outlining potential hazards and associated control measures to be applied. 3) Ensure that employees are trained on safe work procedures.
