Text For Citation: 01 Item/Group: 001 Hazard:
Section 5(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to a roll over hazard: (a) On or about 05/05/2012, employees working at the paving project on Dawkins Road near Caputa, SD, and operating the Hyster C530A bi-directional wheeled packer, serial #A91C2444V, were exposed to the potential for serious injury or death created by machine roll over. Among other methods, one feasible and acceptable abatement method to correct this hazard is to: 1. When it is determined that a road surface has no shoulder with a steep adjacent ditch, the road edges should be tamped rather than packed to prevent a packer from entering the ditch and rolling over. 2. Although currently not required, it is highly recommended that asphalt packers of this type currently owned by the employer be equipped with ROP cabs and seat belts to provide employee protection in the event of roll over.
