29 CFR 1926.416(a)(3): Before work began, the employer did not ascertain by inquiry, direct observation, or by instruments, whether any part of an energized electric power circuit, exposed or concealed, was so located that the performance of the work could bring any person, tool, or machine into physical or electrical contact with the electric power circuit. Temple Universitys Kornberg School of Dentistry, Corridor 2COR5 Before work began, Lex Electric did not ascertain whether any part of the shared neutral conductor for the corridor lighting circuit was completely de-energized and made safe. An employee was exposed to 277 volts AC when working on the shared neutral for that circuit. The shared neutral was energized via a lighting circuit that had not been de-energized. On or about 5/30/2012. Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.
