29 CFR 1915.13(b)(3)(ii): Continuous ventilation was not provided at volumes and flow rates sufficient to ensure that the concentration(s) of toxic, corrosive, or irritant vapors were maintained within the permissible exposure limits (PEL) and below Immediately Dangerous to Life and Health (IDLH) levels during cleaning or cold work. (a) Aboard the F.V. Clipper Epic, starboard companion way deck; On or about May 17, 2012 an employee died of overexposure to carbon monoxide gases produced from gasoline powered pressure washer engine exhaust. The 8 hour TWA exposure was approximately 555.9 ppm carbon monoxide, which is 11.12 times the OSHA PEL of 50 ppm. The employee was pressure washing paint chips off of the wheel house on the deck and the area was enclosed in plastic and not ventilated.
