29 CFR 1910.333(a)(1): Live parts to which an employee could be exposed were not deenergized before the employee worked on or near them: (a) 11130 King Drive, Franklin Park, IL - On or about May 4, 2012, a 480V electrical panel was not de-energized before employees performed work. An arc flash occurred when an employee was pushing metal fish tape through and came in contact with live bus bars.
