29 CFR 1926.502(d)(16)(iii): Personal fall arrest systems, when stopping a fall, were not rigged such that an employee could not free fall more than 6 feet (1.8 m) nor contact any lower level: a) At the facility, on the roof: On April 19, 2012 the employer provided employees a fall protection system consisting of lanyard and rope connected to a horizontal lifeline located on the roof centerline approximately 30 feet from the edge, exposing employees to a free fall in excess of 6 feet in the event of a fall through a roof opening.
