29 CFR 1926.454(a): The employer shall have each employee who performs work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards. a) Jobsite: The employer did not provide training to an employee who erected and worked from a scaffold built with wood beams supported by an A-frame step ladder, stair steps, and a window sill. Employee could not recognize hazards associated with scaffolds, and was exposed to hazards, including but not limited to: scaffold was supported by unstable objects, and scaffold supporting components were not capable of supporting their own weight and at least 4 times the maximum intended load. On or about 03/28/12. NOTE: IN ADDITION TO CERTIFICATION OF ABATEMENT, THE EMPLOYER IS REQUIRED TO SUBMIT DOCUMENTATION OF ABATEMENT FOR THIS ITEM. FAILURE TO COMPLY MAY RESULT IN AN ADDITIONAL PENALTY OF $1,000 AS PER 29 CFR 1903.19.
