29 CFR 1910.147(d)(2): The machine or equipment was not turned off or shut down using the procedures established for the machine or equipment. An orderly shutdown must be utilized to avoid any additional or increased hazard(s) to employees as a result of the equipment stoppage. (a) Roggen Farms, 36401 Weld County Road 24 1/2, Roggen, CO 80652; On and before April 11, 2012, the exposing employer did not ensure that machines or equipment where shut down utilizing procedures established for the machine. Specifically, the hammer mill was locked out at the on/off switch by using a physical device to prevent switch on the stop from topping out, meaning machines main source of power was not secured. Condition exposed employeed to the unexpected energization of the machine during servicing.
