Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recogn ized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to crushing hazards: a)In warehouse North "A": On or about December 29, 2011 and at times prior to; the employer fa iled to provide a clear view of the designated path of travel for the powered in dustrial trucks, exposing employees to crushing hazards while materials are chec ked into the warehouse. One feasible and acceptable method to correct this hazar d would be to install mirrors on the forklift that would allow for unobstructed vision to the rear during backing or to provide spotters to safely marshal the f orklifts to the rear so that a clear view of the path of travel can be maintaine d and allow for the operator can observe for other traffic, personnel and safe c learances as prescribed by the "Safety Standard for Low Lift and High Lift Trucks" ANSI B5 6.1-2009, paragraph 5.3.6. Additionally, the employer could establish a safe are a that was separate from the forklift operating areas where the checker/clerk em ployees could perform their duties free from forklift struck-by hazards.
